An emerging report slams regulators’ response to the ongoing metal-on-metal hip implant debacle. The just-published report appears in the British Medical Journal (BMJ) and accuses regulators of allowing hundreds of thousands of patients who underwent surgery for implantation with metal-on-metal hip implants, of being unknowingly exposed to dangerous, toxic substances.

A joint effort of the BMJ and BBC Newsnight, the report accuses United Kingdom and Untied States health regulators of permitting patients to remain unaware of the danger and alleges that hip replacement manufacturer DePuy Orthopaedics was aware and worried about the possible patient harm linked to devices, even as it touted these products, noted Medscape. British Regulators Say Some Metal-On-Metal Hip Implants Need Lifetime Monitoring

British regulators are calling for lifetime monitoring for patients who received some metal-on-metal hip implants. Testing includes monitoring of exposure to toxic metals. Britain’s medical regulator made the announcement, said The Chicago Tribune, calling for patients who received large-head implants—a head diameter larger than 36 millimeters—saying that they should receive blood tests for metal ions and undergo magnetic resonance imaging scans (MRIs) if they have increased metal levels or are exhibiting adverse symptoms, said the Medicines and Healthcare Products Regulatory Agency (MHRA). A critic of Medtronic Infuse says the product may cause male sterility. Long linked to controversy, the spine growth product was the focus of a just-released study conducted by a Stanford University surgeon.

Aspirin, Plavix Equally Effective in Patients with Blocked Leg Arteries

For those who suffer from peripheral artery disease (PAD), it appears that Aspirin is just as effective as Plavix (clopidogrel) when it comes to treating painful symptoms. According to a new study, Aspirin is just as beneficial for alleviating leg pain that occurs while walking. The results debunk the previous notion that Aspirin would potentially worsen PAD patients’ condition. PAD is a condition in which the peripheral arteries become blocked and narrowed due to the buildup of plaque on blood vessel walls. This tends to occur most in the legs and pelvis. PAD can result in decreased blood flow to the limbs, blood clots and if left untreated, gangrene and amputation. According to the American Heart Association, people with PAD are four to five times more likely to experience a heart attack or stroke. The primary symptom, referred to as intermittent claudication, is pain that arises in the legs while walking that usually ceases upon rest. In the midst of an ongoing, global debacle concerning DePuy’s metal-on-metal ASR hip implant devices, The New York Times reports that DePuy Orthopaedics executives discussed the ASR failure one year before the recall was issued.

It seems that a vice president at DePuy, a unit of Johnson & Johnson, wrote in a company email that the FDA refused to approve the articular surface replacement (ASR) device and that a review of DePuy studies revealed that the device failed early in what was described as “significant” numbers, which meant that patients would have to undergo revision surgery, said The Times. A group of Irish patients are planning a lawsuit over DePuy ASR hip implants. DePuy Orthopaedics is a unit of Johnson & Johnson.

Debate Rages Over Treatment Of Metal-On-Metal Hip Patients

Although it is fairly well established that metal-on-metal hip implants are not all they were originally promised to be, the debate continues on how to treat patients implanted with these devices. Among the headline-making devices are DePuy Orthopaedics’ two ASR hip implant devices and and an all-metal version of its Pinnacle hip replacement system.

Research studies just presented at the annual American Academy of Orthopedic Surgeons confirm some contractions. For instance, British researchers discussed how serum chromium or cobalt ion levels of 7 parts per billion (ppb) or less should be sufficient to initiate a conversation for immediate revision surgery. Yet, said MedPage Today, an American study presented at the same meeting revealed no correlation between ion levels and tissue damage in those patients who underwent revision surgery to remove metal-on-metal devices.
